Regarding Nursing Home Transition, LIFE participants must be referred to their LIFE provider and CHC participants must be referred to their CHC Managed Care Organization (MCO) for NHT assistance. For the purposes of making referrals according to MDS Section Q guidelines, the MCO or LIFE provider is to be considered the LCA for individuals who are enrolled in CHC or LIFE, respectively.
